you don't need to apologise with all the new update here crap it was more than reasonable for you to be suspicious. As far as I know it's just the pill making them lactate there's no way to make them visibly pregnant as far as I know

think that's coming in a later update not sure what one
There's a few of the endings where the characters end up pregnant, but none that I know of during gameplay.
