when you get to the hospital don't say anything flirty to the doctor just tell him straight out where you are and what your name is. Then when jeff comes be demanding and then tell him what happened to you. Then if you want to leave with the doctor, tell him that jeff came and the leave with him.

I dont think what you say about your family effects that, but you have to tell the doctor that you're fine and be chill about the eye. You have to have like 10 points for him to not kill you.
So basically, when the doctor asks how you are say that you're fine and then when jeff comes be rude and tell him what happened he will just inject you with something instead of slitting your throat.
Flirt with him since your first interaction, then pick Horror and Slasher. Once you get past the 'being killed' point, just keep insulting and taunting him without showing fear.
so, once jeff leaves after injecting your IV thing with whatever you tell the doctor what happened when he left instead of lying and saying you were asleep the whole time. Then he asks if you want to leave with him and you say yes. After that jeff will kill you both in the elevator.
