You basically fold the two sides in. The last column on the front page becomes the front cover, the first column becomes a part of the opening spread for the booklet. The inside of the booklet is generally the details for play. (That's how I've been doing most of my tri-fold games.)

What about the middle page?  Are the first & last pages folded together?  Sorry, I don't fully understand.

The back is folded onto the middle, and the front is folded so that it wraps around the whole thing.
