Patreon is only for those who want to buy the game but can't purchase it on Itch. It's just an option. As far as I know, purchases via PayPal are still available here, the problem is only with credit cards at the moment. So no, you don't have to buy the game again, you'll have it in your Itch library forever and will receive all upcoming updates.
