Update: seems to happen after a hammer is used on a known death pill. My guess is the logic to determine when to use the mouth clamp is relying on a list of death pills, but this list isn't getting updated properly after the hammer is used.
thanks for the bug reports! It most probably is that. I thought I updated the list of known pills but must have missed it. Thanks!! (Edit) It was definitely this. Both the hammer and mouth clamp didn't update their list of death pills. Thanks for finding this!!!
