Crazy Streamer Lady will trigger when you reach enough Views, Followers, and Subscribers. (I do not know the exact number of each requirement.)
To avoid it from triggering is to just "Not Streaming at all during your playthrough."
You can avoid getting killed by her if "Jun has an Advanced AI, has at least both Arms, and has Sympathy about 200 or more" (I do not know the exact number for each requirement for Jun to save you from getting killed).
