Could be a fun idea. c;
Not-showering, leaving yourself covered in various fluids, does increase your exposure level in-game, as well as eliciting various comments from NPCs you pass by. But ye, not currently much more than that, I think.

(another good thought, but too much coding.) "addicted" Npc's actively interact with you more, and tend to try and assist you in many ways, even giving random gifts. if they walk across you in a fight maybe they could join visibly, or in text and give you an extra turn dealing their damage, or a hit weaker than yours.
