I bought this module and started playing this today. First time solo playing an rpg and mothership. 

Great vibes so far! One thing i don't quite grasp yet is the incident report form. I had a trip and a fall so i marked it as A3 in the table. So when i have another incident eg. B3 i have a compounding effect. Do i just take a panic test as a result then?

That's basically it! The point of it is to create natural repercussions that are tied to what's occurred already.

In more detail, if when you add a new incident you roll the same value as another incident in the table, it indicates there is a repercussion relating to one of them. You choose which one is causing a new issue and then determine how that presents itself (the semiotic standard table may help if you're struggling with the 'how'). Your character would then deal with this new development (but will need to check PANIC at the very least).

Another quick question: are you meant to listen to the audio recording or read the entry fully upon entering a new section? I interpreted it as recordings at the entrance of a section and one at the end. But that's maybe reading too much into it? It feels better to listen to the whole thing before exploring the section yourself.

Yep, you're expected to get the full entry when you enter a section. The idea is that there are two memocomms, one at each end, but they both share the same local storage, so you can hear all the recordings made in that section on either unit. Hopefully that clears things up!
