It depends, sometimes it's destroying the head, sometimes there is a large dent, the small dent in head that you showed on old video would be pretty underwhelming considering the size of the hammer so the effect is stronger.
Also npcs grab their throat sometimes and you can disable extra blood from main menu in the recent version
