Nah, there's definitely one set that goes to level 4. Think it's at the bottom.
It unlocked when I picked up the janitor gloves on floor two there are no stairs leading to level four from three
Been a while since I touched the rat dungeon, but yeah. Level two has stairs to levels three and four. Bottom of level two has stairs to level 4.
