It does not work, if you use capital letter.

And it does not work to simply exchange the E with an e, if you already have it open with E.

But for this exclusion the adult setting in settings is better.

I am not sure what you mean. If you use "Adult" instead of "adult", the exclude filter will not work. 

About the settings, there is meta information in a project. A project is classified as not suited for minors or the workplace. Or not. This classification corresponds to  https://itch.io/user/settings :

An "adult" game does not need to have the tag adult or erotic. It needs to have the meta information.

Most adult games either do have the tag adult or erotic, but not all of them. Use the user setting to filter out adult content.

(The setting is ignored, if you manually select certain tags. Like literal adult. And erotic.)
