I'm excited for the game's release based on what I've played so far.

I am a bit confused/disappointed by the map layout though. Why is World 1 further away than World 3? I assume you can play levels in any order?

Will there be a health upgrade available? Two health is not much.

As for the location and numbering of the worlds. There really is no impact in the order you complete them since its up to you to decide in what order you want to play them, the world is basically open. The health is always consistent at one heart, but you can take 3 hits before failing a level, and your health refills between levels unless you're playing Retro mode.
