Thanks for the kind words! To answer your questions:
For Crazy Otto mode, whenever you're making your selections for a custom game, just press the O key on your keyboard. You'll see a little Otto icon appear at the bottom. Keep in mind, Otto mechanics will be persistent across all visual styles, but the Otto sprites only show up in Retro visual mode.
As for the request, I'll keep that in mind! At some point in the future, I would love to completely rebuild Pac-Man 37 with a bunch of new features, but it'll take some time to do. This would be a fun feature to add.
