Honestly, all I did was fight one of those snow moths, If I recall correctly. I haven't had the issue since but, I'll try to recreate it, so I can figure out what went wrong!
I have also gotten this eyes on forehead glitch. For me it happens at the end of the dialogue with Hanako where she tells you to go purify the crystal in the mountains. Version 1.3.5 Edit: Glitch fixed itself after kicking Kara's ass in Chilly Ridge.
