Once you've talked to Katie at the gym (and she gives you the gems), you will get a new icon in the top left.
Click on that, and you can drag gems around to assign them to specific poker hands. Only rule is that a gem of strength X can only be assigned to slot X or higher. So a gem designed for a Straight can only be assigned to a hand of Straight and higher
