Yes a d8 for the item list. I forgot to add it in parentheses. Also the two rolls means you roll the die twice to get two items. I’ll make that clearer.

Cool. Only other thing I would suggest is the ladder/stairs frequency be on the same roll as a 50/50? But honestly, having fun with it as it is. My son managed to get 7 STR on level 1 and is having a blast.

Ha, I'm not sure how I'd fit the explanation for the split 1-in-6 roll on to the page. Do you find that one of them happens too often? And what do you think should be more common out of the remaining 5 options?

Sorry it took so long to reply. On one of my playthroughs I got a lot of ladders, which made the dungeon feel tame somehow(?). Silly I know. I have been playing it with more elementary school kids, questions they asked and my answers are below. Might be useful to you for any clarification edits. But with limited space, some openness to interpretation is fine I'm sure.
"If a room has X exits, does that include the door you entered?" No.
"Traps get +1 to the d6 each level?" Yes.
"Can I cast lightning spell and attack too?" No. An attack spell is an attack.
"What about healing spells?" I would say potion + attack is ok, but healing spell takes up your attack turn.
"When rooms touch, can I put a new room's exit into an old room's wall?" Sure. But that cuts down on new directions you can go.
"When I fight two monsters, do I attack one, then defend from both?" Atk M1, Def from M1, Atk M2, Def from M2. One kid preferred Atk M1, Def from M2, Atk M2, Def from M1 which I liked.

These are great, thank you
