Hygiene system is carried over from Dungeon Keep Her game and it's honestly fine. I don't believe it has any effect other then visual one in Raid Her, and you can always use the shower in base camp to raise it to 100 for free. In fact showering the whole duration means you can even harvest an item out of it, too. As for gender suggestion I kinda get that, though I don't think it's necessary - if you find cute enemy, you can always screw them till they like you (make em horny to raise affection, Lustfield equipment is perfect for that) and ask them to join you afterwards - then even as former enemies, they will join your camp after. This means you have lots of options for companions. If you recruit "saved adventurer" you don't like, ask them to "wait" and they disappear after you leave the map.

If anything I'd appreciate option to turn off Futa content entirely as an option because it's not everyone's cup of tea.
