There is a guy who uploaded a guide here explaining how to get the nurse, but she doesn't go to toilet 2F, she goes to Reiko hallway 2, and it can't be unlocked.
I asked you before when you first found her in toilet 2F and she was in the director's room. Did you choose option 1 or 2? If you chose option 2 then she would automatically not be on the asylum ghost list and would not be a real ghost and yeah after banish ghost she will appear in Reiko hallway 2, because you choose option 2 not 1.
