Ettins are present in the mossy expanse part of the map where you also find the outpost and minotaur girl, usually they were in location with a tree for me. Mouth gag can be purchased from strange merchant and it's called "Ring Gag". Care though because if you've been rather generous with sexploits you might be unable to purchase it without becoming her toy ergo triggering game over.
