It's actually based on an old obscure DC war comic called Creature Commandos! I've updated it with the copy suggestions you gave. Failure is that you fail at the action you were trying, however that makes sense to the GM and players. One Page RPGs can't afford to give detailed RP philosophy statements about the nature of failure and suggested consequences, and a lot needs to be implied or brought over from other games. I did add a more explicit mention of what Banes do.
