Format it like this:
Bug #(e.g. Bug #1, Bug #2, etc.)
Bug Description: Visual (what the bug looks like while playing e.g. player left the map bounds)
Bug Description: Action (what you did to encounter the bug e.g. pressed jump & attack at same time)
If it's a game breaking bug,
Go to file explorer and attach the text file from the following location (make sure to change "username"):
C:\Users\username\AppData\Local\Unity\Editor\Editor.log

If you're not sure if it's a game breaking bug, then just add the file anyways.
