There are 3 things that can kill you in the game.
- the monsters attacking you
- the insanity field that makes you mad
- the floors with tentacles, eyes and teeth that can harm you.
So if you died quickly, then a cultist killed you.
if you died randomly, check the insanity bar, it may fill quickly.
