Maybe 'critical orgasms'? 

Have a meter that build as you rutt (or get rutted) and if that is maxed out once your health runs out have an overly dramatic scene where your partner cum like crazy, the whole room shakes, the bed breaks, or even the wall snaps. Make it really hard to raise too, there should be an emphasis on high stats to get this going.

This could open things up for ahegao or mind break, plus could make sex scenes more rewarding once your stats get higher. 

Definitely a fun thought to play around with

Edit: maybe even add an item that gives dialogue where you get shamed for not getting this very high or nutting too soon

Playing build 19 and came across something rather silly:

After already unlocking Orianna I am still finding her coffins. When opened the game continues as if I had opened it for the first time.

This resulted in the following 

-Copies of Orianna will be at Null's location, stacking under their avatar

-these copies can be interacted with and have sex. Each copy will have fresh stats and no position experience, as if they were entirely new.

-each copy will take a position on your drilling team and gain experience at the same time. These can break team limits and grant stacking benefits

-dear fucking gods they all are horny and will form a giant conga line of snu snu almost at the end of every day, please send help my hips are cracking from all the thrusting

I have spent almost 30 days looking for the sheep and she hasn't spawned. I also have a flag on every tile, so that may be stopping her from showing up? I have checked every snow tile and I got nothing.

Plenty of ways to get resources but no where to spend it; could put in some sex options (Maybe random encounters or BOSS battles?) that eat your resources instead of your sex hp. Maybe a base of operations to build up. 

After that the only things that comes to mind are orgies or three ways, you basically nailed a good chunk of what a sex game needs! Another non-sex minigame would be nice, but not needed :)
