You need to apply both the grey lipstick and blush (go to your room to do makeup), then it will continue. Also check the Socials > Side Affair tab for more hints.

There's been a pause on releasing updates recently because I have had to focus on the coding side of things - the project needed re-organizing to better manage in the future, and I needed to fix bugs, set up certain systems and UI stuff. My work the past few months has been to try and get this stuff out of the way so I can focus on writing / gameplay only in the future, which will mean regular updates again.

It is complicated to respond to your comment, especially in regard to the choices you made, because of a mix of me wanting to avoid spoilers, as well as that aspects of the game keep changing as it's developed so I cannot give you a clear idea of where, and where not, the story will go.
The one thing I can point out now is that Dr Mifune is certainly not an ideal version of a Psychiatrist, and very much is a character in an sissy-themed erotic fantasy scenario. More than anything, this game is erotica, with an intention to thrill. I just want to make that clear to set expectations.
