It is definitely much faster than real time, with how quickly the Experiment gets hungry and sleepy and heals their wounds, even considering the high amount of physical and mental strain they are often in. Unless they have an incredibly fast metabolism and healing factor but I don't think that is the case.

I really really wanna see us climbing back up afterwards too, actually getting to work towards an escape to the surface after obtaining the cargo would feel so good in my opinion. As for how that would go exactly, as being able to go up is not as guaranteed as being able to go down, I'm not sure, but I do have some ideas or guesses especially if this experimental technology we are trying to get is going to lead to the events of Gunsaw in some way.
I think 3. with items happens when there is water that is moving back and forth on the ground and for some reason that applies ridiculous amounts of velocity to the items in it, and I don't think is intentional.
As for 4., I believe a body part with very low skin health can get infected if you don't cover it with bandages. Your character tends to communicate this risk to you I think unless there are more pressing matters at hand, and I've noticed it does get infected eventually if you don't wrap it up or heal the skin fast, even if there is no bleeding wound on it or you cleared out an infection on there before.
I fell asleep in a working drill pod, lost my toolbox full of everything, and woke up at layer 4 with almost nothing to my name. But despite that, I made it through.
I even reached Gleeful mood for the first time during this, and without any mind-altering substances. The description of that mood feels so powerful, and it felt quite inspiring in this context too.
Thank you for making this game, Orsoniks. For how much some of the failures and deaths feel sad and sickening, when you do actually prevail it feels that much more inspiring. I hope to carry this tenacity with me to real life.
