I don't know if you're already doing it but there's a way to smooth out a large belly in Koikatsu. It doesn't help with clipping body parts though, large breasts would still clip into it.

Banger game, absolutely love it. Only issue I had was the download version can't resize the window. Not sure if I missed something but I ended up messing around with my display scale I get it bigger. Had no issues with the web version. Seriously, you're on to something here, you've really nailed it with this. Outstanding balance of simplicity and presentation.
