I think that you only get it on your first clear, so after you clear it for the second time onwards you get the "Nothing but blood", I have gotten the the first ending multiple times, but only when I am logged out and on a different computer.
I saw a challenge in the comments and decided to try it for myself, the no orb challenge was pretty hard, but I wanted it to be harder.
