Nothing is missing. The layout convention has most of the book in two columns: a main one for two-thirds of a page width and a sidebar column for my all-too-frequent sidebars and illo overflows. Initial pages for each chapter, however, rely on an even two-columns of main text. Sorry for the confusion.
