Hey! The basic answer to all of these questions is to follow your instincts as a DM. Your ideas would all be acceptable to me if I was playing in a game you were running. I didn’t want to overwrite the rules because this game survives on flexibility between the players and the dm.

Whenever the characters set out on a mission or anything, The GM has them decide on what loadout they want to use: light/medium/or heavy. That determines how many gear slots they have during the mission, but the players don't need to choose the actual gear in their loadout until the moment need it.
