In my experience it occurs if the same ram gets you with hypnosis multiple times in a row. You can also get it by using the masturbate curse in her close vicinity while naked. If she isn't currently using a skill (signified by her glowing) she will approach you and 'help you out'.

The ghost bell abillity that you get from having some or all into kitsune transformations works relatively well, if you can get a few of those before the fight you can stun her repeatedly while using other attacks and techniques on her

Does anyone know where the saves are stored?

Because up until now, whenever an item was required, I could just walk up to it, press the interact key and the item menu would automatically come up. I didn't even know there was a designated item menu until just now, I just thought that was how it worked.

Given that the interact key was the only one that was introduced on screen when the disembodied penis attacked. In any case, sorry for the false alarm.

No one else seems to have mentioned this so I'm not sure if this is new or as a result of something I did, but although I'm able to pick up the vials, I cannot interact with the respective fluid dispensers in order to fill them up. They are sparkling, so I'd presume I'd just need to stand in front of them and press the interact key, but that doesn't work, no item menu or dialogue prompt comes up. Has anyone else gone through this?
