That's intended behavior. To get more height you need to hit something on the way down.
Drop rate's 1/20 for all of them.
That's the grouper, the big red fish that first appears in the Hanging Pits.
The secret area is accessible from the top region of the Extinction Chamber.
If I told you more, it wouldn't be much of a secret.

Sorry for the late response. Do you have information on what other items you had equipped? I'll look into it regardless.

The check for whether it drops is based on whether you have it, so you should be able to get another.
