Ok I think I see what is happening, there was something wrong with the monocle. I went back to the graveyard and it was still there, so I grabbed it again and went to Alexander same issue. I tried a third time and a dialogue box popped up saying I got everything and I am able to see the H scene. I tried restarting the game and replicating the issue but the dilogue box pops up every time now. I think this might have been a weird one off issue that solved itself.

So I collected the hat, monocle, and pipe. When I go to the lizard man he is already wearing them and when I talk to him he asks me if I collected them all and reminds where they might be. Am I missing something? Should I load a earlier save? I got the monocle from the graveyard, the pipe from the surfing game, and the hat from the city. Not sure what to do now.
