(Spoilers)
- The code can be acquired in the electrical substation if you have the knife (crumpled note in inventory). You can then use the code + toilet blastr to get the key for the hather door.
- For the floral sacrifice you need to gather swamp lilies from the wetlands and take them to the alter in the underground sewer area. (Just go full left in the sewers.)

Unfortunately it may be the case that your save is corrupted. There are a few possible reasons for this..
1. I had to edit a few dialogue trees from earlier chapters. If you had previously saved on one of those (very small) branches, then your save would break. (This is my guess as to what is happening)
2. You loaded up a newer version of the game and then loaded up an older version. I'm guessing you probably didn't do this but it would definitely probably break everything.
3. This is a new glitch that I don't yet understand and hasn't been reported.
If it is the case that your other saves still work then #1 is the likely scenario and you'd be fine playing on other files. If every file is corrupted then you'll have to delete your save data via preferences and start over, apologizes! :[
