this game is very RNG based. the mouse guy kills me all the time even though i shine my light on him. and a lot of the mechanics are not implied through gameplay very well like the vent guy with red eyes, i went in the locker when i saw his eyes but he came into my office anyway, so i assumed the locker wasnt the solution, and then when he killed me i come to find out the locker was indeed the solution, so probably dont make him progress forward if you are in the locker to convey that better, if you get in the locker while his eyes are there he should probably just go back entirely with a sound cue. it's decent though, 'it could be better' in a nice way

i would say to remove the tutorial stuff at the beginning, or put it into a alternate section, and replace it with just a simpler 1-screen short message, and make all collectibles and general good things have a white outline, and make all bad things/enemies have black outline, then just explain in the new tutorial that "Things with white outlines are good, things with black outlines will deal damage or kill on impact" or something. also make the insta-kill circles... not insta kill, at least not on easy and normal, and make them knock you back and make you spin out a little bit afterward, and have them pulse red to indicate they're bad too.
