I've had the same issue popup, it only seems to happen in the room behind the silver doors. Reloading the game typically fixes it for me, but not always.

I just encountered a very strange bug, where the orb would phase through random pegs.
It happened just after picking up the Electropegnet and after the corridor event. I ignored the secret passage and while attempting to fire my down the left passage the orb phased through the first couple of pegs and then did a complete U-turn into the right side passage. It was like the orb followed a completely different trajectory than what was displayed on screen.
Other information in case there is a reasonable explanation-
Relics: Strange Brew, Rallying Heart, Ring of Indignation, Shrewd Scales, Well-Done Steak, Ambidextionary and Electropegnet.
Orbs: 3x Stones, 1x Daggorb lvl 2, 1x Bramball lvl 2, 1x Poltorbgeist lvl 2, and 1x Nosforbatu lvl 2 (favorite orb btw).
The pathing on with the Electropegnet also seems a bit odd from when it was patched, like the bombs repel the orbs instead of attract them.
