So the problem with night 4 (and possibly 5) Is that the 1-20 scale for aggression is already at 20 at the beginning of the night. I believe the game is allowing itself to go over the initial aggression, but not below it. This meaning that, even if you didn't let MalO slip by even once, the game sees it as maxed and kills the player. 
This could be fixed by,

A: Lowering the difficulty (19 would be fine)

B: Allow the game to go under the initial aggression (if int aggro is 20, catching her would make it 19)

You are almost correct, at night 5, she already starts at 18 / 20, and increases past 20 as the night progresses (max 21), making her appear on screen more. but there is a seperate meter that increases whenever the player fails to catch her. If that goes over a certain amount, you automatically lose, but it goes down dramatically when the player does catch her. 

That being said, I will lower the difficulty across nights 4 and 5 so everyone can access the rest of the scenes. :)
