I have similar issue.

1. First star is for beating regular mission.
2. Second for beating the  same mission with completely different squad.
3.  Third for beating it with extra objective.

I've beaten both versions of level 4 with various squad combinations and still only got 2 stars (or 1 star above flag as you describe it).
