this is physically unrealistic because there is no through path. and if it is still done, it will be almost instant death, with which it is also painful. even a direct path from the mouth to the anus, which is sometimes found in hentai, in reality will lead to no less painless death. not to mention the fact that many people do not like such scenes and are considered cruel to put it mildly
