To unlock this one, you need to still satisfy the conditions of ALL 4 of the other statues - hope this helps! If you need more of a hint, let me know!

Ah - Bradley needs to have unlocked all 4, as well as still in Samurai class, inflicted with poison and zombie and with HP less than 25%!
