I agree with you, originally I had the ladder hitboxes a lot tighter, and they'd sort of snap you into place as you climbed them (which arguably felt worse lol), so I loosened them up a lot. There's still issues with it though as you said, specifically getting off the top of ladders if you're a few pixels off of being at the very top of the ladder.
