When playing Classic mode, if any of your cats get sick, injured or are going through grief, they will be permanently stuck in the Med Den and will never recover, effectively making even perfectly healthy and capable cats into Elders.
if you're seeing people's cats get hurt and then recover you're seeing them play expanded mode. Cats gaining conditions (getting hurt or sick) on classic mode is a bug
