To gain ego you must be confident. Being more aggressive in your battles or fighting larger than you enemies greatly increases ego. Otherwise, once you take the Lich quest from the Apothecarist she sells two new potions, one of which is an ego potion. Edit: If you get defeated you lose you also lose large amounts of your ego.
