Are you aware that the thrust command sometimes clips the hips through the ground? Also do you think you might add more sexual content into the game beyond just gore?
I personally would enjoy some version of cum flooding the villages. Maybe have system were you have to build up an erection first by doing lewder and lewder things to the villagers. Why not some anal vore too while I'm giving ideas.
