This is like the dark souls of NSFW games. How do you defend against enemies coming from behind? The only way I've found is the special attack which you can only do 3 times per level...
Jump before their about to hit you then move backwards to avoid them.

I also thought it is impossible without special attack but as other said jumping and going back do the job
