No, the titles, or rather quirks, are not all unlockable.

Put the halloween furniture in your room and then check the dungeon at night.


As for Kind/Cruel stuff.
For the time being that has no effect, it might in the future but it probably will be minimal.
Adding that much branching would make the game's progress move slower than a three legged tortoise covered in tar.
Also Arc seems to dislike games that force you to replay the entire game so you can see content that you missed because you picked the "wrong route".
