The .exe doesn't work on its own. You have to upload everything in the Unity build folder (zip them into a zip/rar file), not just the exe. Make sure to test it on its own before uploading.
Also for the future it's less clunky to use a .txt file for simple text info like that, as opposed to a Word document. You could include that in the zip file too.
