Hey! Sorry i couldn't answer straight away. I was doing some research on this question. So here is Japans route!
First of all, be sure to select female gender. If you did already, then restart game anyway. Write your name and select gender once again. Then visit art club and choose "Anime" as an answer there. Since if you don't do it, Japan's route doesn't always work... I have no idea why.
Join art club.
When you make a poster with Japan, select these answers:
"Red-pink"
"Blue"
"Dramatic"
During math project, join Japan, Germany and Austria. Then choose "I agree with Austria"
Draw something during class whener you have a chance (and if it affects Japan, not Italy). When Japan asks what you are drawing, answer "Anime" and let her look.
During break, in the club room, when Japan asks you what kind of games you like, answer "Visual novels"
If you did everything correctly until this moment, Japan will ask "You also like drawing anime, right?". Anwer "Yeah, why?". Then she will ask to collaborate. You should answer "Of course!"
After that just wait until the classes end (choose to draw something during class once again until then).
After class, select "You're finally here! Let's go work on the dating sim!"
Say "Great idea." when Japan proposes to go to the cafe.
In cafe choose "Some boba, please!". After that choose "I hope we can go on adventures like that together.". Then agree to meet up with her after school.
On the next day, during break, you can go to club to meet with Japan. But there won't be any choices there. THOUGH you can give gift there.
After some time, during break in club, Japan will ask to help her with code. Answer "Sure!"
At Japan's house, agree to go out with her this weekend. (You can give gift here)
On weekends you will get a text from Japan. Answer "yes!!!"
When Japan asks what would you like to drink, ask for tea. Then say "You're so fast!"
After that answer "I love a slice of anime!"
After playing the game, answer "I did!". As for the favorite part, choose "I loved everything!".
After Japan tells about never being in relationship, say "I would never make fun of you for that."
When you have an otion to write a love letter, choose to talk to Japan. Then select these answers:

"Peach is pretty good"
When Japan asks to meet in front of the school, choose "Alright..."
That's it! Congrats! If you didn't get the ending, try to restart game once again. Or always fall asleep during class. The reason may be in you being low on energy or something. There is also a second route. At least that's what the list of routes says. There is also a picture of it in game files. I've been trying to find it these past 2 days, but I didn't succeed... I really am sorry :(. All I could get is new dialogs. And even if I had good relationships with Japan there, game still didn't give me a chance to date her anyway. Okay, so Americas route.
spend time with him during breaks like with other characters
On the first day during break he will ask about how are you feeling at this school. Answer "Honestly, you're the coolest person here."
During English test always choose second option. If you did everything right, he will ask you to come over and 'study'. Say "Sure".
At his house ask "Do you like anime?", "Nyan Neko Sugar Girls", "Kawaii Metal is cool, eh?" (You are able to give gift here)
During Math project join America, Russia and Ukraine. Then choose:
"Please calm down, we've got to work on the project, right?"
"Calm down, please"
"Let's use blue"
During break, at schoolyard, stay with America.
In class, when you sit with America, choose to talk to him.
After that, during break, at schoolyard, say "Alright...".
When he asks where Canada could be, choose third option.
During break, at schoolyard, choose "You should tell Canada yourself".
Once Canada and America made up, you can visit America during break at schoolyard and give him a gift.
Basically after they made up, there won't be any choices there. Just hang out with him during breaks and in the end write him a love letter.

Hello! Honestly, I couldn't find it for a long time too. But after your question, I downloaded the game again and im happy to announce that I found it!
Basically its just Italy route but with some changes. You join his club, spend time with him and such. I assume you know how to get his usual route.
Back to the point. The MOST IMPORTANT part of yandere route is: during class, when there is an option to write something and Italy asks you what are you writing, choose poetry. Let him see it. Then he will ask you "Do you believe in destiny?". The answer is "Not really". And this is where it starts
After that you see "two guys with a fluffy dog" (Finland and his buddy). Choose to approach them. Youll see changes right after that choice.
After that you just hang out with Italy as usual UNTIL you come to club and see Grecia instead of Italy. Choose "I want to help".
After that just hang out with Italy until he invites you to see his project. Answer "Im busy, sorry".
No choices matter until you have "Write a love letter to someone" available. It doesnt matter to whom you write it. Just write. Thats it.
(There are also little differences between yea or no at the end)

If you're intersted in playing the whole Italy route, I have some updates. I saw that someone said something about Finland missing. So I decided to investigate Italy route further. This time I wont write a whole essay, just basics. Important stuff: Join Drama Club, during math test support Italy, during lessons choose Italy otions (draw, write). Of course say that you don't believe in destiny. Then stop visiting him at club at all. At first I started to hang out with Finland. Even gave him gifts. Agreed to cook in his house. If he doesn't meet you up, you're on the right way. After that I met Finland during break once, then Russia and afterwards America. Just go elsewhere but club. If Finland, Russia, America or Canada aren't avaible during break, just go to class. If you hang out with side charactes, you might accidentaly get their ending. In the end you write a love letter again. All I mentioned doesn't affect ending, but makes plot more interesting. Im afraid I didn't investigate enough when I writed his guide the first time.
