Have to be identified as a specific gender pronoun, which can be done in the sanctum through the identity tile. And be have respective gender genitalia. Once that's done go to the deserted base, get caught by Mystyra and repeatedly wait. Make sure to save first cause as far as I'm aware you can't actually continue after getting the megaboob/megacock ending. Hope this helps.
