does this mean only slow transitioning male to female exist? as in there are no "already existing" shemales, correct? i said "female to male" because the description read this way. as in, only transitions exist but no pre-set pussyboys/shemales.

As for the player, yes, there is no immediate in between option. You can transition from Male to Female over a short period, start as one or the other, or there is a character who can immediately change you between the two later.
But for NPCs, there is a character that is a woman with a dick that exists which you can interact with.
