The upper chance of success is 8 (80%) because once you get higher than that there's very little point in rolling. The risk just isn't high enough. So instead of getting a higher chance of success you get an increased chance of an exceptional success.
Yes a fantastic chance means you have exceptional success or success with an added benefit when you roll 1 to 4.
The solo table is d36 because it's easier to fit inside one page without shrinking the table down to a tiny font size. It's the same with the Obstacle/Discovery table.
